All our patient records are kept on computer. We can assure patients of complete confidentiality. All practice staff are bound by a confidentiality agreement. Your rights are also protected by the Data Protection Act. We may at times be required to share your clinical data with the Primary Care Team.
Concerns about confidentiality can be addressed to our Caldicott Guardian, Dr J Pywell via the Practice Manager.
You have the right to access your computerised and paper medical records upon written request. You will be required to show some form of identification. Under GDPR we can charge if requests are excessive and repetitive.

The practice conforms to the requirements of the Freedom of Information Act 2000. Full details are available on www.foi.nhs.uk. If you do not have access to the internet please ask at reception and a copy will be made available.
Patient’s Rights and Responsibilities are as defined in the Patient’s Charter, a copy of which is available upon request from NHS England.
